Modification Form (Property) 

• Submit request online; or
• Print form and return via mail/USPS.

Homeowners are responsible for maintaining their property in accordance with DeKalb County Code of Ordinances and the Circlestone governing documents.

• Most, if not all, external changes must be approved by the Board to ensure properties are maintained according to established standards as outlined in the governing documents.
• This form is REQUIRED and must be APPROVED before any exterior modifications or repairs are made to any property in the Circlestone Community. 
• Failure to obtain written approval prior to work beginning will result in a fine up to $1500.00 per violation.
